This case involved a crew maintaining backside pressure while a pressure pumping company was conducting hydraulic fracture treating operations. Due to the wellbore construction hydrocarbons were allowed to enter the annular space. Once hydrocarbons were in the annular space they entered the backside pressure pumping unit. Once inside the unit ignition occurred injuring the backside operator.
This case involved a noise heard by the flowback crew. While investigating the noise the number one oil tank exploded launching the supervisor, who was on top of it, quite some distance. The supervisor lost his leg in this incident.
This case involved a workover crew utilizing snubbing assist to pull out of the hole after drilling out frac plugs. The snubbing assist company utilized single joint elevators in the laydown operation. The single joint elevators were designed for API collared tubing rather than the PH-6 tubing being utilized. A joint of tubing was dropped from the single joint elevators striking a work over rig operator in the head causing serious injury.
This case involved a leak in a zipper manifold. Pumping was shut down and the white leg was nippled down to replace a ring gasket. While nippled down the white leg fell upon a pressure pumping hand. Causing serious injury.
